The Developmental Learning History Form helps build a profile of a child’s development from age 0 up to age 12. Used as part of a psychological assessment, this profile helps to identify significant learning and/or behavior difficulties of concern at school or at home, and provide some clues as to why these difficulties have arisen.
The Developmental Learning History Form serves as a precursor to a more extensive report that sets out a diagnosis in full, which in turn provides short term prescriptive suggestions for managing or improving the issues identified, both at school and at home.
